If you would like to save time and complete the intake form prior to talking with a LOCATE Referral Specialist, print out and then fill in the form below. Please print clearly.

After completing it, you can fax it to 410-385-0561 or, you can scan the completed form and email it to intakes@marylandfamilynetwork.org. If you prefer, mail it to:

A LOCATE: Child Care Referral Specialist will contact you within 3 business days during regular LOCATE hours between 8:30 am and 4:00 pm.
